Athlete's Acknowledgment of Team Rules

I hereby fully understand that my violation of any team rules may result in the loss of my athletic privileges or in suspension from the team. The coach will determine the length of time of the suspension.

Parent's Acknowledgment and Permission

When a parent signs his/her name below, he/she acknowledges that he/she is aware of the rules the athlete will follow during the athletic season. Parental acknowledgment is requested so that there can be no misunderstanding among parent, coach and athlete about what is expected of the student/athlete. The athlete, by signing above, agrees to abide by the training rules for the duration of the season.
